This course invites the audience to reflect upon various aspects of safety barriers by expanding on the concepts introduced by Erik Hollnagel in his 2007 paper “Risk + Barriers = Safety?” (Safety Science, Vol. 46, Issue 2)
Learning Objectives: By the conclusion of the course, participants will have learned:
- Critically analyze the 2021 British Airways Boeing 787 landing gear collapse utilizing the concept of physical, functional, symbolic, and incorporeal barriers (Hollnagel, 2007)
- Present strategies based on current safety science research to assist safety practitioners in devising more-effective barriers in response to identified risks
- Elicit a discussion on ways of monitoring the performance of implemented barriers.
Instructor Bio
Lucca Carrasco Filippo, a former helicopter pilot and flight instructor, is a safety training and promotion specialist at NetJets Europe, where he is responsible for delivering safety management system (SMS) and safety investigation training to flight crew members, maintenance teams, and office staff. He holds multiple qualifications in SMSs, risk management, and human factors and has extensive experience in conducting occurrence investigations. Lucca is also a regular contributor to the Flight Safety Foundation magazine AeroSafety World.
